To get your medical card in Utah, first schedule a visit with a qualified medical provider who will assess if medical cannabis is right for you. Use our Find a QMP feature to locate one near you.
Start by registering at id.utah.gov, if you don't already have an account. Once you've verified that account, go to evs.utah.gov and log in using the same username and password as your id.utah.gov account. There you'll see a link to begin the patient application process.
