What to bring and how to prepare for your appointment:

• Before arrival:
  • If renewing: Log into the EVS to resubmit your application.
  • If new: Create your EVS account before your visit.
• Identification:
  • A valid state ID or U.S. passport is required.
  • EVS cannot be updated using IDs or licenses expired more than 6 months.
• Device Access:
  • Bring a smartphone or device to access the Utah EVS system during your appointment.

Medical cannabis cards are valid for up to one year, subject to your QMP’s discretion.
